Responsibilities:
- Assisting clients with activities of daily living (bathing, dressing, grooming, toileting, eating, transferring, and mobility).
- Meal preparation
Performing light housekeeping. - Medication reminders & Assistance.
- Engaging clients in meaningful conversation, socialization, and activity while providing personal care assistance.
- Following client-specific care plans as well as keeping all necessary documentation current.
- Observing changes in physical, emotional, mental, or behavioral conditions, unusual symptoms, accidents, injuries, or unusual occurrences, and reporting promptly to a licensed nurse.
- Perform other duties as assigned.
